Barossa Village has recently added new names to its Community Home Care Team, located at The Lodge on Scholz Avenue in Nuriootpa. 

Clare Pietsch was recently appointed as the Community Care Team Leader, and she explains the various roles that help to deliver aged care to people in their homes.

As Community Care Team Leader at Barossa Village, what is your role?

My main responsibilities are to support my team and to manage enquiries about Home Care Packages the Commonwealth Home Support Program, aged care services available in our region and provide assistance with accessing My Aged Care.

Who else is in your team?

We have four Care Coordinators, two Registered Nurses, a Commonwealth Home Support Package Coordinator and a Support Officer. Our team is based here in the Barossa.

What does a Community Care team do?

Care Coordinators work with Home Care package clients to discuss their needs and goals of care, helping their clients to safely remain in their own home.  Coordinators set up a care plan which meets the needs and wishes identified by their client.

Registered Nurses provide various nursing services to clients in their own home and can also provide assessments for a range of health and wellbeing domains.

The Commonwealth Home Support (CHSP) Coordinator manages new client referrals to this program, which is an entry level to aged care services, focussing on clients maintaining independence and links within their community.CHSP can help with activities of daily living, such as domestic assistance, home maintenance and social support.

The Support Officer provides assistance to Care Coordinators by setting up and rostering services for clients and managing and supporting the Contractors who deliver the servicers.

How far do you travel to provide home care services?

Barossa Village provides care throughout the Barossa and Light Council areas and Gawler:

  • Barossa: Angaston, Eden Valley, Lyndoch, Moculta, Mt Pleasant, Nuriootpa, Penrice, Springton, Stockwell, Tanunda and Williamstown

  • Gawler

  • Goyder – Eudunda, Point Pass, Robertstown

  • Mallala, Wasleys, Two Wells
